Behavioral Assessment

Hello August! The start of a new month is a great reminder to reflect and refresh before life gets busy. This week, we are listing five areas related to our day-to-day health. In each area, use a scale of 1-5 (1 being poor and 5 being great) to honestly rate your overall status. Additionally, take a moment to identify one or two habits in each area. 

Using a check-in scale of 1 – 5, rate:

  • Nutrition
  • Sleep
  • Stress management
  • Intake of alcohol and high calorie drinks
  • Physical activity

So what do you think of this exercise? Any valuable insights or takeaways? What do you feel is your healthiest area at this time? What is one area you would like to focus and be more intentional about this month?
